New Patient Registration
Patients wishing to register with the Practice are required to complete and sign a New Registration Form, provide documentation proving they have lived in the country for move than six months and a telephone number where the patient may be contacted. They are also invited for a New Patient registration check to update their records.
Summary Care Record
This is a secure Electronic record of important information about your health and will only be available to health care professionals providing you with treatment in England and they will ask your permission before they look at this information. This means that if you have an accident or become ill, medical staff treating you will have access to important information about your health. The record will contain information about the medicine you take, any allergies you suffer from and any bad reactions you have had to any medicine. The surgery is supporting the Summary Care Record however you do have a choice and you can change your mind at any time:

Yes I would like a Summary Care Record - You dont have to do anything

No I dont want a Summary Care Record - Ask at reception for an opt out form when you register with us, you must fill it in and hand it back to the receptionist
If you do nothing we will assume you are happy with these changes and create a Summary Care Record for you. Children under 16 will automatically have a Summary Care Record created for them unless a parent or guardian opts out on their behalf.
New Patient Check
When registering with the Surgery we will ask you to book a “New Patient Check”. This will be with the Health Care Assistant or Practice Nurse and we request that you bring a urine sample to the appointment. The Practice Nurse/Health Care Assistant will run basic health checks and tell you more about the surgery.
